Terrorism Forms

The Terrorism Risk Insurance Act (TRIA), signed into law in 2002 and extended in 2005, provides for a transparent system of shared public and private compensation for insured losses resulting from acts of terrorism. The program is implemented by the U.S. Treasury Department.

Insurance carrier policy mandates that all prospective policyholders complete and sign a terrorism form as proscribed by the specific insurance carrier. All of our carrier's terrorism forms can be found below.
